Coach Biographies

Blazer Team Managers - Spring Registration

MYSL - Refer to MYSL website for updated forms

You need to have the following in packet for the BBSC office to register your team:

• 4 copies of your roster, updated for spring. Be sure that player codes are changed to “E” for existing players. Goalie notation is required

• Player pass cards

• MYSL Team Affiliation sheet – be sure to complete the division in which you want to play

• MYSL Team Registration Sheet – be sure to note any player transfers or releases and include the proper forms

• Roster additions must have: player contract, proof of birth, 2 1x1” pictures

• $200 Registration Fee per player

(Packets with the affiliation sheet and registration sheet have been given to your team’s coach)

Premier – Refer to MSPSL website for updated forms

You need to have the following in the Packet/Envelope provided by MSPSL: Envelopes have been distributed to your coach.

• 3 copies of your roster, updated for spring. Be sure that player codes are changed to “E” for existing players.

• Goalie notation is required.

• Player contract for any new player

• Transfer/release forms as needed

• Team Information Sheet (from website)

• Envelope filled out

• $200 Registration fee per player

Be sure you register your players thru the MSPSL on-line system. Refer to their website for process.
